Care Manager Program

The St.Vincent Care Manager Program is dedicated to providing support to patients and families when there is uncertainty where to turn for necessary services. By evaluating an individual’s level of cognitive abilities, along with his or her health, emotional, financial and legal needs, our Care Manager will create an individualized plan of care to provide information and options in order for the patient and their family to make the best decisions possible. Acting as advocates for the elderly, we assist clients and his or her family with promoting well-being, managing crises and navigating through our health care system. The program is a fee-for-service program that has been designed to:

  • Conduct a comprehensive individual assessment to determine level of functioning and need of resources.
  • Develop and implement a personalized plan to achieve short-- and long-- term goals.
  • Assist client and family in finding the best available care options.
  • Screen and arrange for needed special services and programs.
  • Collaborate with physicians, other health professionals and local resources to ensure that the client receives cost effective quality care.
  • Ensure financial stability including examining health benefits, supplemental insurance plans, Medicare, and Part D programs.

We also provide evaluations for your specific needs to help the patient maintain independence. Such services include:

  • Safety of living environment
  • Functional ability
  • Medical history
  • Medications
  • Nutrition
  • Mental status
  • Financial and legal issues
