Rehabilitation Services

St.Vincent Home Health Care and Rehabilitation Services are provided for a wide variety of diagnosis with the goal of making the transition from hospital to home easier for the patient and family by getting you back on your feet as quickly as possible.

We offer comprehensive rehabilitation services to help patients achieve an optimal level of functional independence. Our services include:

  • Physical Therapy: Improving the body by building strength, improving mobility and developinggreater confidence through special exercises and physical training.
  • Occupational Therapy:Assisting patient to become as independent as possible in daily living skills such as dressing and bathing through special exercises and activities.

Speech Therapy: Helping patients develop skills that enable them to think and express themselves more clearly.Speech Therapy also helps patients with swallowing disorders and can teach specialized exercises and training to swallow safely.

Our therapists first perform an evaluation and then formulate a personalized treatment plan for you, which may include the following:

  • Therapeutic exercises to increase trunk and extremity strength, joint range of motion, balance, endurance and general reconditioning.
  • Prescribing individualized home exercise programs.
  • Bed mobility and transfer training, to allow you to move about their environment as freely as possible.
  • Ordering necessary assistive devices or medical equipment.
  • Massage to decrease pain, muscle spasms, and/or edema, and improve flexibility.
  • Lymph edema wrapping.
  • Educating you and your family about their diagnosis and prognosis.
  • Teaching family and caregivers necessary skills so they may better provide for you while allowing you to remain as independent as possible.
  • Home safety and fall prevention evaluation and education.
  • Instructing pain management techniques post surgical, acute injury or chronic pain.
  • Various modality applications such as ultrasound, heat packs, ice packs, TENS unity as appropriate.
  • Therapeutic treatment.

Consider a rehab referral when a patient’s function abilities decline due to an acute illness or progression of a chronic debilitating condition including:

  • CVA
  • Falls
  • Musculoskeletal conditions
  • Neuromuscular conditions
  • Fracture and joint replacement aftercare
  • Pneumonia
  • COPD
  • Renal disease
  • Language re-training
  • Cognitive-linguistic re-training

Hospitalization is not necessary to have home therapy services, however it must be orderered by a physician
