Telehealth Monitoring

At St.Vincent Home Health, we offer the Philips Telehealth system to help patients take a more active role in their health and stay connected to their healthcare provider. It’s a simple, cost-effective way to monitor patients with chronic illnesses within the comfort of their own homes.

It only takes a few minutes a day, and a patient can take their measurements by themselves. If a patient’s measurements fall outside desired limits, the central station – connected through a phone line - alerts the monitoring nurse and appropriate actions are taken to ensure the wellbeing of the patient at home.

Consider Telehealth if your patient has:

• CHF/Heart failure
• COPD
• Diabetes
• Hypertension
• Frequent admission to the hospital
• Open heart surgery

Telehealth provides a comprehensive range of personal monitoring options to help people continue to live in their own homes - maintaining their independence, personal security and their quality of life. The patient can feel safe in their home and monitoring is available 24 hours a day, 7 days a week 365 days a year.

There is no charge for the Telehealth service as it is part of receiving skilled care through St.Vincent Home Health. Telehealth is also available, for a fee, through our Private Duty program when a patient is discharged from skilled care.
