Concerns about Your Care
Should you have concerns regarding the care you receive during your hospital stay, please contact one of the following associates:
- Department Director: Generally available Monday through Friday, 8 a.m. to 5 p.m.
- Practice Facilitator: Generally available Monday through Friday, 24 hours a day
- Administrative Representative: Available 24 hours a day, seven days a week
Please ask your nurse to reach a department director or the practice facilitator. To contact the administrative representative, dial "0" for the operator.

- Fraud, abuse or false claims
- Environmental, health and safety issues
