The home health care team provides services that contribute to the patient's welfare and shares health skills and information so that the patient and caregivers can become more independent.

Persons are considered for Home Health Care when their medical, nursing, therapy, and social needs are intermittent and capable of being safely met within the confines of their own home. Home care does not provide ongoing, 24-hour, around the clock or respite services.
