How to Start Therapy
If you think you might need or benefit from physical therapy or occupational therapy, ask your doctor. If he/she agrees, they can write you an order for therapy.
Just like X-rays and medication, you need a doctor’s order for therapy. Then, simply call us at (812) 276-1282 to make an appointment.
What to Bring to Your First Appointment
You will need to supply your insurance card and therapy prescription.

Please enter at the northwest entrance located at the corner of 23rd and “Q” Street. Take the elevator to the first floor, we are located right outside of the elevator.
