HIV Care Coordination

HIV care coordination services provide access to health coverage, patient assistance programs, and other basic needs.

The program is coordinated by a licensed clinical social worker who offers individual, group and family counseling and facilitates a weekly support group for HIV-diagnosed patients and their families.
