Senior Care Navigation Program

The Senior Care Navigation Program provides professional and compassionate guidance for seniors age 65 and older. It is a liaison and resource center for seniors and their families who are making difficult healthcare decisions. This service is especially useful helpful when there is a concern about falls, when the individual is unable to manage household tasks, when there is no local advocate or someone to provide routine followup and coordination of care, or when there has been a recent discharge from the hospital after repeat visits.

The Senior Care Navigation Program can assist when a loved one is part of a nursing home or assisted living program. The staff can help address any questions or concerns the family may have in collaboration with facility staff members, acting as a liaison and advocate for the family and their loved one.

The program offers clients a holistic assesment with screenings for cognition, mood, nutrition and home safety. We can help with medical referrals and coordinate health care, with routine follow-up communication.

No physician referral is required. Anyone can be a part of the program. Some of our inpatient or outpatient referral sources include medical doctors or nurses, case managers, social workers, assisted living or nursing home providers, health access workers, caregivers, attorneys, home care agencies and the client's family.
